29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings , and openings through which conductors enter were not effectively closed: Drill site: On or about October 18, 2016; unprotected flexible cords running through the electrical box openings of three trailers were not protected from abrasion. This condition exposed employees to electrical shock hazards in the following instances; a) Trailer #MH 0802 N.O.V. Solid Control living quarters had an unprotected flexible cord running through the electrical box opening. b) Trailer #MH 0816 Tool-Pusher living quarters had an unprotected flexible cord running through the electrical box opening. c) Portable toilet trailer used by drill site crews and subcontractors had an unprotected flexible cord running through the electrical box opening.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: Drill Site: On or about October 18, 2016; unused openings in electrical boxes were not effectively closed. This condition exposed the employees to an electrical shock hazards in the following instances. a) Trailer #MH-0816 - The electrical box at the Tool-Pusher's living quarters had an unused opening on the bottom. b) Trailer #MH-0663 - The electrical box at the Directional Driller living quarters had an unused opening on the bottom. c) Trailer #MH-0357 - The electrical box at the Mud Engineer living quarters had an unused opening on the bottom. d) Trailer #MH00878 - The electrical box at the Company Man living quarters had an unused opening on the bottom.
