29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. On or about May 02, 2023, Nabors Drilling Rig #X16 site, TanMar Rental LLC equipment exposed employees to trip and fall hazards to due employer not ensuring that no opening in a stair rail system exceeded 19 inches in the following instances: 1) Two in the front open staircase on Trailer #MH40747 2) One in the front and two in the back open staircase on Trailer #MH0487 3) One in the front and one in the back open staircase on Trailer #MH463 4) One open staircase in back of Company Representative Sleeping Trailer. 5) Two open staircases on Mud Lab Trailer. 6) One open staircase in front of Company Representative Trailer.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about May 02, 2023, Nabors Drilling Rig #X16 site, TanMar Rental LLC equipment exposed employees to electrical shock hazards in the following instances. 1) Behind the Crew House Trailer, two extension cords (25-foot Red/black AWG 16 / 25-foot orange/black 14 AWG) were plugged into each other (daisy-chaining). 2) Behind the Recycle Trailer leading to the honey pot, a 25-foot orange/black 14 AWG extension cord was laid in mud and water.
