29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  At the location of 1651 Blue Jacket St. in Overland Park, KS an employee engaged in fascia installation with the use of a working platform with unprotected sides was exposed to fall hazards of approximately 10 feet from the eave to the ground below. The employee was exposed in that the working platform was missing a fall protection system to keep him from falling.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the location of 1651 Blue Jacket St. in Overland Park, KS an employee engaged in roofing activity on a 10:12 pitch roof was exposed to fall hazards of approximately 8 feet from the eave to the ground below. The employee was exposed in that the ladder in use did not extend 3 feet above the access point.
