The employer had set up warning lines on the flat roof, but the warning lines were touching the ground in several areas and were not maintained/supported in such a way that the lowest point was no less than 34 inches from the walking/working surface.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about November 3, 2016, Bash Roofing Company had an employees exposed to struck-by hazards on the head, while working on the ground under employees on the roof as they tossed roofing material off the roof, on a commercial construction site in Urbana, Illinois. Bash Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.100(a), which was contained in OSHA inspection number: 1064785, citation number 1, item number 1 and was affirmed as a final order on June 30, 2015, with respect to a workplace located at 712 Park Lane Drive, Champaign, IL 61820.
