29 CFR 1910.157(d)(6): The employer did not provide portable fire extinguishers for Class D hazards which are required in those combustible metal working areas where combustible metal powders, flakes, shavings, or similarly sized products are generated at least once every two weeks: a) On or about 10/27/16 in the Grinding area; a portable fire extinguisher for Class D hazards was located behind the cutting laser more than 75 feet away from the grinding area where aluminum, carbon steel, and stainless steel dust, powder and shavings were generated on an almost daily basis exposing employees burn hazards. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about 10/27/16 in the machine shop; the interlock on the access door to the Okuma LT-10M lathe was not functional allowing employees access to the rotating ( 4,500 rpms) tooling head exposing them to struck by or caught on hazards. b) On or about 10/27/16 in the machine shop; an interlock on the access door to the Mazak 200M lathe was missing allowing employees access to the rotating (4,500 rpms) tooling head exposing them to struck by or caught on hazards. c) On or about 10/27/16 in the machine shop; an interlock on the access door to the Mazak VTC 160 A mill was missing allowing employees access to the rotating ( 10,000 rpms ) tooling head exposing them to struck by or caught on hazards. d) On or about 10/27/16 in the machine shop; an interlock on the access door to the Mazak VTC 200B mill was missing allowing employees access to the rotating ( 10,000 rpms ) tooling head exposing them to struck by or caught on hazards. e) On or about 10/27/16 in the machine shop; an interlock on the access door to the Mazak Nexus 1500 vertical mill was missing allowing employees access to the rotating ( 10,000 rpms ) tooling head exposing them to struck by or caught on hazards. NO ABATEMENT CERTIFICATION REQUIRED
