29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by the rotating tooling heads on the lathes and mills; a) On or about 05/02/17 in the machine shop; an interlock on the access door to the Mazak 250 lathe was by passed when the pin was not secured and installed in the interlock with the door open allowing employees access to the rotating (4,500 rpms) tooling head exposing them to struck by or caught on hazards. b) On or about 05/02/17 in the machine shop; an interlock on the access door to the Mazak VTC 160 A mill was by passed when the pin was not secured and installed in the interlock with the door open allowing employees access to the rotating ( 10,000 rpms ) tooling head exposing them to struck by or caught on hazards. ABATEMENT DOCUMENTATION REQUIRED
