29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:  Employees engaged in manufacturing activities in the plastics department were exposed to amputation hazards in that training was not provided regarding the prohibited use of gloves while working near the following pieces of equipment which utilized rotating bits:   a)  Employees engaged in flashing removal activities at the job made router in the plastics department.  b)  Employees engaged in reaming activities at the bench top drill press located in the plastics department.

29 CFR 1910.179(l)(3)(iii)(d): Pendant control stations were not kept clean, and/or their function labels were not kept legible:  Employee engaged in material handing activities using the overhead crane and hoist in the east side of plant were exposed to struck-by hazards in that the crane and hoist pendant control was not equipped with legible markings.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Employees engaged in manufacturing activities in the plastics department were exposed to amputation hazards while removing flashing and reaming out holes on parts in that the following pieces of equipment were not equipped with machine guarding:  a)  The bench top drill press in the plastics department was not equipped with a guard to prevent employees from contact with the rotating bit and chuck assembly.   b)  The job made router in the plastic department was not equipped with a guard to prevent employees from contacting the rotating bit.
