29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:   a) Job site, at/near 1612 Graham Court, Pittsburgh, PA: On or about November 2, 2016, during residential construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 13 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, at/near 1612 Graham Court, Pittsburgh, PA: On or about November 2, 2016, during residential construction activities, the employer did not ensure that the ladder used to gain access to/egress from the top of the first floor was properly set up in that it extended approximately 16 inches above the top of the first floor wall and was visually about even with the top of the flooring above the wall.
