29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a) 1 Montella Circle, Limerick, PA, Worksite - on or about April 12, 2017, an employee was working from a pump jack scaffold where an extension of the working platform of approximately four feet was placed on one end, which extended the platform length approximately seven feet beyond its nearest support.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) 1 Montella Circle, Limerick, PA, Worksite - on or about April 12, 2017, an employee was working from a pump jack scaffold, approximately 17 feet above a lower level, without being protected from falling to that lower level.   Carmelo Construction, Inc.  was previously cited for a violation of a similar occupational safety and health standard, 29 CFR 1926.451(g)(4)(i) which was contained in OSHA inspection number 1178420, citation number 1, item number 2a and was affirmed as a final order on February 21, 2017, with respect to a workplace located at 1819 Ginnoda Street, Philadelphia, PA 19130.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) 1 Montella Circle, Limerick, PA, Worksite - on or about April 12, 2017, employees were installing siding to a residential home project while working on steeply pitched roof, approximately 17 feet above grade, without utilizing any means of fall protection.   Carmelo Construction, Inc.  was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1191483, citation number 1, item number 1 and was affirmed as a final order on January 18, 2017, with respect to a workplace located at Lot 47, David Drive, Newtown, PA 18940.
