29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. (a) On or about November 8, 2016 and at times prior, employees were exposed to struck by hazards where the horn on forklift 64018 was not functioning in the dock. (b) On or about November 8, 2016, employees were exposed to struck by hazards where the parking brake on forklift 69116 was not functioning in the dock.
