29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about November 7, 2016 at 2845 East County Road 177; fall protection: The employer failed to ensure employees receiving a bundle of trusses from a crane pick on the third level ceiling area of a new Towne Place Suites Hotel had their harness attached to the life lines which exposed them to falls of 42 feet 5 1/2 inches to the ground below. b) On or about November 7, 2016 at 2845 East County Road 177; fall protection: The employer failed to ensure employees receiving a bundle of trusses from a crane pick on the third level ceiling area of a new Towne Place Suites Hotel had their harness attached to the life lines which exposed them to falls of 10 feet 1 3/4 inches to the inside floor.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: a) On or about November 7, 2016 at 2845 East County Road 177; 6 foot step ladder: The employer failed to ensure a six foot step ladder was properly used in that it was leaning against a wall to gain access to the ceiling area by using all the steps (including the top two which are not to be used) exposing them to fall hazards.
