29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training:  On or about November 10, 2016, at a worksite located at 699 North Huntington Street, Medina, OH 44256, employees were exposed to fall hazards when the employer did not provide retraining for employees using ladders. Employees were observed standing on the top two rungs of Werner 6 foot step ladders while conducting carpentry activities and received ladder training on May 1, 2013.

On or about November 10, 2016, at a worksite located at 699 North Huntington Street, Medina, OH 44256, an employee was exposed to fall hazards of approximately 10 feet when the employer did not ensure fall protection system/device was utilized by employees.     LM TRIM  CARPENTRY LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501 (B)(11), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 897117, CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON May 28, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 35680 DETROIT ROAD, AVON, OH 44011.
