29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    A) LM Trim Carpentry LLC on or about April 4, 2018 at the address of  4121 King Road, Sylvania, OH: The employer did not ensure employees were protected from a 30 fall to the level below. The exposed employee was installing OSB sheeting from a top of the structure, without an adequate form of fall protection in place.      The LM Trim Carpentry LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(1), which was contained in OSHA inspection number 1190594, citation number 2, item number 1 and was affirmed as a final order on January 5, 2017, with respect to a workplace located at 699 North Huntington Street, Medina, OH.      The LM Trim Carpentry LLC was previously cited for a violation of an equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 897117, citation number 1, item number 1 and was affirmed as a final order on May 28, 2015, with respect to a workplace located at 35680 Detroit Road, Avon, OH. 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.    A) LM Trim Carpentry LLC, on or about April 4, 2018, at the address of 4121 King Road, Sylvania, OH: The employer did not ensure employees were wearing appropriate eye protection while operating a pneumatic nail gun to secure OSB sheeting to the roof of the structure. One employee was exposed to potential flying object hazards while operating the pneumatic fasteners.
