29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on November 5, 2016, when three employees stripping old roof materials and installing new waterproof underlayment on a 4:12 pitch, low slope roof of a residential structure located at 10 Warner Place, Palm Coast, FL 32164 are exposed to an 8 foot fall to the ground below in that they are not protected from the hazard by any means of roof anchor points, lanyards, personal fall arrest systems, guardrails or safety nets.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 997948, citation number 1, item number 1, and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 228 Melissa Ray Drive, Jacksonville, FL 32225.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 996902, citation number 1, item number 1, and was affirmed as a final order on March 24, 2015, with respect to a workplace located at 4324 Queensway Drive, Jacksonville, FL 32257.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 983753, citation number 2, item number 1, and was affirmed as a final order on September 30, 2014, with respect to a workplace located at 11645 Fallingleaf Trail, Jacksonville, FL 32258.
