The employer fails to assure effective protection against falls as required by the standard, as demonstrated on November 5, 2016, when three employees stripping old roof materials and installing new waterproof underlayment on a 4:12 pitch, low slope roof of a residential structure located at 10 Warner Place, Palm Coast, FL 32164 are exposed to an 8 foot fall to the ground below in that they are not protected from the hazard by any means of roof anchor points, lanyards, personal fall arrest systems, guardrails or safety nets.  Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 997948, citation number 1, item number 1, and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 228 Melissa Ray Drive, Jacksonville, FL 32225.
