The employer fails to assure effective protections against falls as required by the standard, as demonstrated on February 3, 2017, when a male employee wearing a straw hat, white striped shirt, tennis type shoes, and tan cargo type pants, is working on the south and west slopes of a residential roof with a 6:12 pitch at 30 Milliken Lane, Saint Augustine, Florida, and is exposed to an 19 feet fall hazard to the ground in that he is not attached to a personal fall protection system. The employer fails to assure effective protections against eye and face injuries as required by the standard, as demonstrated on February 3, 2017, when an employee is working on a residential roof at 30 Milliken Lane, Saint Augustine, Florida, without eye protection while using pneumatic nail guns and is exposed to eye injuries from flying debris or nails. 29 CFR 1926.1053(b)(1): Where portable ladders are used for access to an upper landing surface and the ladder's length allows, the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Employees using ladders to access the roof are not protected from fall hazards by extending the ladder 3 feet above the landing.    The employer fails to assure effective protections against falls as required by the standard, as demonstrated on February 3, 2017, when 6 employees using a ladder to access a residential roof at 30 Milliken Lane, Saint Augustine, Florida, are exposed to a 6 feet fall hazard to the lower level in that the ladder does not extend 3 feet above the landing.
