Employees were exposed to an explosion hazard and/or struck-by hazard.

29 CFR 1910.147(a)(2)(ii)(B): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: 1) Production Area: On or about September 19, 2016, employees were conducting maintenance of the bender machine (model T50168) and did not use normal shutdown procedures. The authorized employee got his right hand caught-in the machine and did not utilize the written LOTO procedures that were in place before troubleshooting. An employee sustained an amputation hazard to right index, middle and ring finger.
