29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from severe cuts or lacerations, severe abrasion, or punctures:    (a) Roll Mill Room:  On or about 11/17/2016, an employee was performing clean up work on the roll mill and was not provided with cut-resistant gloves for use when cleaning the knife area.

29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s):    (a) Roll Mill Room:  On or about 11/17/2016, the roll mills were not provided with a safety instruction sign to warn employees to use a guarding device such as, but not limited to, the nip point barrier pipe, wash up stick, wooden v bar guard or contoured screen at the feed roll and middle roll in-running nip point, to prevent contact with the inrunning rolls by the operator's hand.  Signs should contain wording such as, "DO NOT START WASH UP UNTIL ALL IN-RUNNING NIP POINTS ARE PROVIDED WITH A GUARDING DEVICE."     (b) Roll Mill Room:  On or about 11/17/2016, the roll mills did not have safety instructions posted to educate employees performing cleaning of the equipment and rolls with "Safe Wash Up Procedures" to instruct them in the hazards associated with clean up such as, but not limited to, proper use of tools, safe distance to open the feed rolls, and use of rags.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts:     (a) Roll Mill Room:  On or about 11/17/2016, an employee was exposed to the in-running nip point of the feed rolls on the small TechniMask 3 Roll Mill which was not provided with an adequate guard or guarding device such as, but not limited to, a wash up stick with interlock.
