29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) At the facility (Pharmacy Dept.): A relocatable power tap (RPT or surge protector) was not directly connected to a permanently installed branch circuit receptacle, instead, it was connected to an extension cord which was plugged into an outlet located inside the cupboard under the sink. Employees were exposed to electrical/fire hazards on or about November 16, 2016. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.
