On or about 4/13/17, on site, for employees using a 110v heat gun on a daily basis. The heat gun's electrical cord is plugged into a flexible cord. The flexible cord is then plugged into a junction box hanging above the work station. The junction box was not installed onto a wall, as it should be used.

On or about 4/13/17, on site, for employees using a 110v heat gun on a daily basis. The heat gun's electrical cord is plugged into a flexible cord. The flexible cord is then plugged into an electrical box hanging above the work station. The flexible cord is a substitute for fixed wiring since the employee's work station is permanent.
