29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Employees conducting machine servicing, such as adjusting the speed of the conveyor system on Line #15, were exposed to amputation hazards and energy isolating devices were not physically located and operated in such a manner as to isolate the machine or equipment from the energy sources, and lock out or tag out devices were not affixed to each energy isolating device. The employer did not require that employees follow the sequence outlined in 29 CFR 1910.147.

29 CFR 1910.219(m)(2)(i): Expanded metal, sheet or perforated metal, and wire mesh was not securely fastened to frame. On Line #15, the north side metal guard covering the belt and gear drives was not secured to the frame or otherwise configured to prevent the removal of the guard while the drives were operating.
