29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) South Side Party Wall - Employees engaged in carpentry operations were exposed to fall hazards of approximately eight (8) feet when working around an unguarded floor hole. Observed on or about 02 JUNE 2016. b) West Side of Jobsite - Employees traveling on and off the jobsite were exposed to a floor hole that was not covered or guarded properly. Observed on or about 02 JUNE 2016.
