29 CFR 1926.959(d)(1): Mechanical equipment was not operated so that the minimum approach distances, established by the employer under � 1926.960(c)(1)(i), were maintained from exposed energized lines and equipment: On or about December 1, 2016 at the substation job site, 120th Ave. SW, S of 40th Ave SW, Cunningham, KS, employees engaged in construction work from an articulating JLG boom lift were exposed to electric shock hazards in that the employer did not ensure that minimum approach distances from energized equipment were maintained.
