29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Line #4, Labeler C - An employee amputated a finger and burned his hand when it was drawn into the machine by an ingoing nip point; on or about 11/16/16 at 59-02 Borden Avenue, Maspeth, NY 11378. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: (a) At the worksite - 59-02 Borden Avenue, Maspeth, NY 11378 An employee amputated a finger and was hospitalized on or about November 16, 2016. The employer reported the incident two days later on November 18, 2016.
