29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:      a) On December 16, 2016, on the first floor main lobby area, large square holes greater than 24 inches were located in and around the walkways. Exposing workers to trip and fall hazards.        b) On December 16, 2016, at the main entrance ramp from the side road, holes in the metal ramp walkway were not covered. Exposing workers to trip and fall hazards.

29 CFR 1926.1424(a)(2)(ii): 29 CFR 1926.1424(a)(2)(ii): To prevent employees from entering hazard areas, the employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas and/or did not train each employee to understand what these markings signify:      On December 16, 2016, on the north side of the construction site, employees walked beneath a crane in operation pushing a cart with a compressed gas cylinder. There were no barricades, warning signs, or observers in place. Exposing workers to caught-between and struck-by hazards.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:      a)  On November 13, 2016, an employee was struck by steel beam and required medical treatment to include prescription medication. The employer did not record the work-related injury on the OSHA Form 300 or an equivalent.    b) On November 15, 2016, an employee struck by steel beam and required medical treatment to include prescription medication. The employer did not record the work-related injury on the OSHA Form 300 or an equivalent.    c) On November 16, 2016, an employee was struck by a piece of heavy metal, and injured their ankle which required medical treatment to include prescription medication. The employer did not record the work-related injury on the OSHA Form 300 or an equivalent.    d) On November 16, 2016, an employee cut their right index finger while operating a saw resulting in an amputation which required medical treatment to include prescription medication. The employer did not record the work-related injury on the OSHA Form 300 or an equivalent.    e) On November 17, 2016, an employee operating a drill press, drilled through his thumb and required medical treatment to include sutures, shots, and several follow-up visits for an infection. The employer did not record the work-related injury on the OSHA Form 300 or an equivalent.    f) On November 19, 2016, an employee was struck on his left finger by a piece of steel and required medical treatment to include prescription medication. The employer did not record the work-related injury on the OSHA Form 300 or an equivalent.    g) On December 2, 2016, an employee stepped on a nail with his left foot and required medical treatment to include prescription medications and treatment for infection.

29 CFR 1904.39(a)(2):  The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:   On October 19, 2016, an employee had a finger amputated as a result of an occupational related incident, and the employer did not report the amputation to OSHA.
