29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  On or about 17 November 2016, approximately 12:00 PM, an employee sustained a work place injury from a ground level fall that resulted in hospitalization, which was not reported to the Occupational Safety and Health Administration (OSHA) within (24) twenty-four hours of the incident. The employee was hospitalized the same day as the injury and remained in the hospital until 6 December 2016, when they passed away due to complications of the injury.
