29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   On or about 12/8/2016 in the production, maintenance, warehouse, and packing areas at 4350 Peachtree Industrial Blvd, Ste 100 Norcross, GA:  Where employees performed maintenance and or service on equipment such as the mixers, raw material process machines, water pumps, grinder, henschels, and fillers equipment, did not have clear and specific energy control procedures.  The employer failed to address all of the energy sources in the procedures such as but not limited to electrical, pneumatic, and mechanical, water, gravity, and heat energy sources.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 12/8/2016 in the production and maintenance areas at 4350 Peachtree Industrial Blvd, Ste 100 Norcross, GA: Where employees were repairing and servicing malfunctioning equipment, troubleshooting, setting up, clearing, unjamming, and adjusting equipment no periodic inspection was performed annually on the energy control procedures for the following equipment;  a.       Raw Material Mixing Equipment two (2) each, b.      Raw Material Processing Equipment & Pumps two (2) each, c.       Raw Material Grinder Equipment one (1) each, d.      Grinded Material Henschels Additive Equipment one (1) each, and e.      Finished Toner Filler Equipment three (3) each.
