29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.     Northern Repair and Welding facility located at R15300 Ringle Ave. Ringle, WI: On or about December 14, 2016, employees were exposed to 4 feet and greater fall hazards while working on elevated surfaces, including but not limited to, tanker type trailer equipment when perimeter guardrails were not provided.  Other feasible alternative means of fall protection such as temporary guarded scaffolding or the use of personal fall protection systems were not used to protect employee from falls.

If such hazards are present, or likely to be present, the employer shall: (i) Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  Northern Repair and Welding facility located at R15300 Ringle Ave, Ringle, WI: On or about December 14, 2016, employees were exposed to fall hazards and respiratory hazards when the employer's assessment of the workplace did not identify the hazards or corrective measures to prevent:  A) falls from elevated working surfaces;  B)  exposure to hazardous chemicals containing Isocyanates;  C: exposure to hazardous environments while working in permit required confined spaces.

29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:    Northern Repair and Welding facility located at R15300 Ringle Ave, Ringle, WI: On or about December 14, 2016, the employer had not implemented a written respiratory program and painters were exposed to metal coating operations containing Isocyanates and solvents.  Employees were provided with respirators but they had not been medically evaluated, fit tested, or trained to properly wear respirators, and welders welding stainless steel were not provided with appropriate type respiratory protection to protect them from Hexavalent Chromium fume.

29 CFR 1910.252(b)(4)(iv): Lifelines. Where a welder must enter a confined space through a manhole or other small opening, means shall be provided for quickly removing him in case of emergency. When safety belts and lifelines are used for this purpose they shall be so attached to the welder's body that his body cannot be jammed in a small exit opening. An attendant with a preplanned rescue procedure shall be stationed outside to observe the welder at all times and be capable of putting rescue operations into effect.  Northern Repair and Welding facility located at R15300 Ringle Ave. Ringle, WI: On or about December 14, 2016, the employer did not ensure that employees welding in confined spaces were  provided with an outside attendants to monitor their work inside those spaces and to provide attendant with a preplanned rescue procedures and equipment to retrieve workers quickly and safely from the space in the event of an emergency.

29 CFR 1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.    Northern Repair and Welding facility located at R15300 Ringle Ave. Ringle, WI: On or about December 14, 2016, the employer did not ensure that area in front of and on the both sides of the electrical breaker panel located in the tool room was kept clear at least 3 feet.  Five gallon buckets  of fluid and other equipment was stored in front of electrical panel that obstructed safe access.       Northern Repair and Welding was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 924554, citation number 1, item number 2, and was affirmed as a final order on 3/19/14, with respect to a workplace located at R15300 Ringle Ave. Ringle, WI 54471.
