29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) On or about December 14, 2016 and at times prior to that date, for the employees exposed to amputation hazards where the Pacific 500 ton hydraulic press, model 500PF, Serial No. 5558 was operated utilizing the foot control pedal to form parts in the fabrication department located at 3401 First Avenue North, Fargo, North Dakota.
