29 CFR 1910.212(a)(2): Machine guard(s) on machine(s) were so installed that they offered accident hazards in themselves:  (a) Research and Development Center (R&D Center), Room 185SW - On or about January 5, 2023, the employer failed to ensure that machine guards at R&D Center did not create additional hazards and thereby exposed employees to caught in and struck by hazards. An employee made incidental physical contact with fencing causing it to shift toward a powered rotating driveline axle shaft that the fencing was intended to guard against. The chain link was caught, ripped from the frame and became entangled on the end of the protruding rotating shaft. The employee's hand was also caught, and their arm was drawn into the rotating equipment resulting in several limb bone fractures and avulsion of a thumb.  Abatement documentation is not required of this item.
