29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Facility, 385 Tremont Ave, East Orange, NJ 07018  a)  The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of PPE such as but not limited to goggles, gloves and chemical resistant work clothing in the kitchen where employees use corrosive chemicals including Ecolab Greasestrip Plus and Value Clean Oven and Grill Cleaner, to clean and degrease the kitchen.    b)  The employer did not assess the workplace to determine if hazards are present which necessitate the use of  PPE such as but not limited to gowns, gloves, goggles, surgical masks and filtering facepiece respirators for the housekeepers that go into hospital rooms with patients with communicable diseases. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Facility, 385 Tremont Ave, East Orange, NJ 07018  The employer did not implement their written hazard communication program for employees who use corrosive chemicals to include but not limited to Ecolab Greasestrip Plus and Value Clean Oven and Grill Cleaner to clean and degrease the kitchen.   VA New Jersey Health Care System was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.1200(e)(1,)  which was contained in OSHA inspection number 994648,  Notice number 1, Item number 1,  and was affirmed as a final order on 4/9/2015, with respect to the same workplace located at 385 Tremont Ave, East Orange, NJ 07018.  Violation occurred on or about 12/13/16
