29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:        a) In the welding area, on or about 1/31/2017: The employer did not administer a hearing conservation program for employees who performed welding, grinding, sanding, cutting, and milling of parts. Monitoring performed by OSHA showed an employee exposed to continuous noise at a level of 75.38% of the dose of the permissible exposure limit (PEL). The equivalent dBA for the 75.38% is approximately 87.9 dBA calculated as the 8 hour time weighted sound pressure level. Sampling was conducted for 411 minutes. A zero increment was included for the remaining 69 minutes of the workday.       Abatement certification must be submitted for this item.

29 CFR 1910.107(b)(5)(iv): Space within the spray booth on the downstream and upstream sides of filters were not protected with approved automatic sprinklers:        a) In the spray booth, on or about 3/6/2017: Space within the spray booth was not protected by an automatic sprinkler. Employees sprayed flammable products including Sherwin Williams Envirolastic 940 dtm, Envirolastic 840 dtm, and Duraplate 235.           Abatement certification must be submitted for this item.
