29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: (a) On or about December 20, 2016 for employees exposed to a fall hazard of approximately 11.5 feet while installing roofing materials on a 4x12 pitch roof located at 1200 Collins Avenue in Mandan, ND. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 985581, Citation 1, Item 1 and was affirmed as final order on September 26th, 2014, with respect to a workplace located at 2900 North Washington Street in Bismarck, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 943545, Citation 1, Item 1 and was affirmed as final order on January 7th, 2014, with respect to a workplace located at 3650 West Princeton Avenue in Bismarck, North Dakota.
