29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Seal and Acoustics Department: On or about December 19, 2016, the employer did not ensure the use of eye protection, such as, but not limited to safety glasses, for employee(s) who were exposed to eye hazards from chemical spray processes using 3M Neoprene High Performance Contact Adhesive 1357L (n-Hexane, Acetone, Methyl Ethyl Ketone, Polychloroprene), an eye irritant. b) Seal and Acoustics Department: On or about December 19, 2016, the employer did not ensure the use of eye protection, such as, but not limited to safety glasses, for employee(s) who were exposed to eye hazards from debris generated by the use of a compressed air gun for cleaning processes.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: Warehouse: On or about December 19, 2016, and times prior thereto, employees were exposed to hazards of undue physiological burden from the mandatory use of Sperian and 3M half-mask tight fitting respirators and N-95 dust masks while performing spray paint processes, chemical adhesive application and sanding tasks. The employer did not establish a written respiratory protection program or implement provisions of the program, such as but not limited to medical evaluations, fit tests, selection and maintenance of respirators, proper use of respirators, or ensure that employees who use respirators did not have facial hair in mask seal areas.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes: (a) Fiberglass Department: On or about December 19, 2016, employees were exposed to chemical hazards from the application of Frekote 700 NC (Dibutyl Ether, Naptha, Octane) and Lacquer Thinner FPL50-53 (Toluene, Xylene, Acetone, Ethyl Benzene),which are skin irritants, and the employer did not require the use of chemical resistant gloves. (b) Seal and Acoustic Department: On or about December 19, 2016, employees were exposed to chemical hazard s from the spray application of 3M Neoprene High Performance Contact Adhesive 1357L (n-Hexane, Acetone, Methyl Ethyl Ketone, Polychloroprene), a skin irritant, and the employer did not require the use of chemical resistant gloves. (c) Fabric Department: On or about December 19, 2016, employees were exposed to chemical hazards from the spray application of StarStuk NS38 Adhesive(Acetone, Hexane, Toluene, Heptane), a skin irritant, and the employer did not require the use of chemical resistant gloves. (d) Shade Department: On or about December 19, 2016, employees were exposed to chemical hazards from the application of Bestine (Heptane), a skin irritant and permeator, and the employer did not require the use of chemical resistant gloves.
