29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections have been performed. The certification did not identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection: a) ACME Manufacturing Co, Inc. at 4661 Monaco Street, Denver, CO 80216: On and before December 19th, 2016, the employer did not certify that periodic inspections of energy control procedures were conducted for machines such as, but not limited to: - Threaders (x7); - Roll formers (x2); - Straight form and roll (x2); - Uncoilers (x5); and the - Automated sheer sheet line This condition exposed employees to an increased risk of amputation hazards.
