29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard in the workplace:    (a)   Zenetex LLC, at Buildings 680 and 674, Hill Air Force Base, UT: On and preceding 12/20/16,  the employer did not identify and evaluate the respiratory hazard in the workplace.  Employees used pneumatic orbital sanders to remove corrosion and paint, including but not limited to "L04" paint, from aircraft or aircraft parts.  Employee exposure to air contaminants generated by the sanding was not determined.  This condition potentially exposed employees to a respiratory hazard.    (b)   Zenetex LLC, at Buildings 680 and 674, Hill Air Force Base, UT: On and preceding 12/20/16,  the employer did not identify and evaluate the respiratory hazard in the workplace.  Employees sprayed paint, including but not limited to "L04" paint, onto aircraft or aircraft parts.  Employee exposure to air contaminants generated by the painting was not determined.  This condition potentially exposed employees to a respiratory hazard.

29 CFR 1910.134(f)(6): QLFT may only be used to fit test negative pressure air-purifying respirators that must achieve a fit factor of 100 or less:    (a)   Zenetex LLC, at Buildings 680 and 674, Hill Air Force Base, UT: On and preceding 12/20/16,  the employer did not ensure that QLFT was only used to fit test negative pressure air-purifying respirators that must achieve a fit factor of 100 or less.  Employees performed maintenance of aircraft.  Employees were required to wear respiratory protection when sanding aircraft.  Employees were provided 3M models 6700, 6800, 6900, or 7800 full facepiece tight fitting air purifying respirators.  Employees received qualitative fit tests using irritant smoke.  The employer did not ensure that wearers of full facepiece tight fitting air purifying respirators were provided quantitative fit tests.  This condition potentially exposed employees to a respiratory hazard.

29 CFR 1910.134(k)(1): The employer did not provide effective, comprehensive, understandable, annual respirator training to employees who are required to use respirators:  (a)   Zenetex LLC, at Buildings 680 and 674, Hill Air Force Base, UT: On and preceding 12/20/16,  the employer did not provide effective, comprehensive, understandable, annual respirator training to employees who are required to use respirators.  Employees performed maintenance of aircraft.  Employees were required to wear respiratory protection when sanding or painting aircraft or aircraft parts.  The employer required employees to wear 3M models 6700, 6800, 6900, or 7800 full facepiece tight fitting air purifying respirators or 3M model H-410 hooded supplied air respirators.  The employer did not ensure that all employees were provided annual respirator training.  This condition potentially exposed employees to a respiratory hazard.

29 CFR 1910.1026(d)(1): The employer, who has a workplace or work operation covered by this section, did not determine the 8 hour TWA exposure for each employee exposed to chromium (VI):    (a)  Zenetex LLC, at Buildings 680 and 674, Hill Air Force Base, UT: On and preceding 12/20/16, the employer did not determine the 8 hour TWA exposure for each employee exposed to chromium (VI).  Employees used pneumatic orbital sanders to remove corrosion and paint from aircraft.  The 602 paint contained strontium chromate and barium chromate.  Employee exposure to hexavalent chromium was not determined.  This condition potentially exposed the employees to a respiratory hazard.    (b)  Zenetex LLC, at Buildings 680 and 674, Hill Air Force Base, UT: On and preceding 12/20/16, the employer did not determine the 8 hour TWA exposure for each employee exposed to chromium (VI).  Employees painted aircraft or aircraft parts.  The 602 paint contained strontium chromate and barium chromate.  Employee exposure to hexavalent chromium was not determined.  This condition potentially exposed the employees to a respiratory hazard.

29 CFR 1910.1200(e)(1): Employer had not developed, implemented or maintained at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met:    (a)  Zenetex LLC, at Buildings 680 and 674, Hill Air Force Base, UT: On and preceding 12/20/16,  the employer had not developed, implemented or maintained at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f) and (g) of this section for labels and other forms of warning, and safety data sheets will be met.  Employees performed maintenance of aircraft.  The employer had not developed a written hazard communication program with regards to labeling and safety data sheets related to hazardous chemicals in their work area, including but not limited to the following:    1.  LO4 paint; and  2.  "602 Primer" (containing strontium chromate and barium chromate).
