29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Pallet Band Saw: On or about December 21, 2017 and at times prior, employees were exposed to unexpected machine start up while changing blades on the pallet band saw. The employer did not ensure that employees followed the requirements of a cord and plug machine in that employees were not unplugging nor controlling the hazardous energy of the cord and plug connected machine.
