29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    (a) Lifetime Construction LLC at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing and controlling employer did not ensure that jobsite inspections were conducted by a competent person. Five employees of the roofing subcontractor were working on a 18/12 steep pitched roof without the use of fall protection, hard hats, and safety glasses. This condition exposed the roofing subcontracting employees to fall hazards and struck by hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) Red Roofing at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing and controlling employer did not ensure employees were wearing hard hats to be protected from the hazard of falling objects on a steep roof.  Employees were using forks to tear off roof shingles from a 18/12 steep pitched roof. Employees were working beneath each other on the roof, and on the ground where falling debris presented a hazard to the employees. This condition exposed employees to head injury from being struck by falling tools and materials.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) Red Roofing at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing employer did not ensure employees were wearing safety glasses when using pneumatic nail guns while sheathing, and while using forks to tear off wood shingles. This condition exposed employees to the hazards of projectiles to the eyes and from being struck by sharp objects.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a) Red Roofing at 135 E 4th Ave Delta, CO: On 12/13/2016, the exposing employer did not ensure employees were protected from falling through the use of appropriate fall protection.  The employees were sheathing and tearing off shingles on a 18/12 steep pitched roof without the use of fall protection. This condition exposed employees to fall hazards of approximately 35 feet.
