29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) Exterior of Building - An employee without any eye protection was exposed to eye injury while engaged in framing activities using a pneumatic nail gun, Condition noted on or about 12/28/16.   Express Construction Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 952855 and 1079116. In regards to Inspection# 952855, citation number 01, item number 01 at a workplace located at 396 W. Hudson Ave., Englewood NJ 07631 which was affirmed as a final order on 4/7/14, In regards to Inspection # 1079116 citation number 02, item number 1 at a workplace located at 1240 River Road, Edgewater NJ 07020 which became a final order on 09/15/2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system :  a- Roof Level: An employee was not wearing a personal fall arrest harness and was not secured via an anchor point exposing the worker to falls of approximately 20ft to the concrete below. Condition noted on or about 10/08/2016.   Express Construction Carpentry was previously cited for repeat violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR  501 (b)(13), which was contained in OSHA Inspection Number 952855 and 1079116.  In regards to Inspection Number 952855, Citation # 01, Item # 02, was affirmed on 4/07/2014 to a workplace located at 396 W. Hudson Ave, Englewood, NJ 0763.  In regards to Inspection Number 1079116, Citation #02, Item # 02, was issued on 08/18/2015 to a work place located at 1240 River Road, Edgewater NJ 07020. This citation became a final order on 09/15/2015    .

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) Exterior of Building - Rear Roof : An employee was exposed to a fall hazards of approximately 20 feet as the worker carried a full sheet of plywood up an aluminum extension ladder to the roof while engaged in sheathing activities. Condition noted on or about 12/28/16.  b) Exterior of Building- Extension Ladder: An employee carrying a partial sheet of plywood on an extension ladder was exposed to fall hazards of approximately 15ft to the ground below while engaged in sheathing activities. Condition noted on or about 12/28/2016.     Express Construction Carpentry was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.1053 (b)(22):  which was contained in OSHA Inspection Number 1079116, Citation # 1, Item # 1, issued on 08/18/2015 with respect to a work place located at 400 International Drive, Mount Olive, NJ 07828 which became a final order on 09/15/2015.
