29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Building Exterior: An employee was observed positioned at the base of an extension ladder where a pneumatic nail gun was being use by another employee directly above, thus exposing the worker below to an overhead impact hazard as head protection was not utilized, on or about 3/28/2019  The Express Construction carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1200082, citation number 1, item number 1, and was affirmed as a final order on 4/3/2018, with respect to a workplace located at 590 Oak Ave Saddle Brook NJ 07663

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) Exterior of Building - An employee without any eye protection was exposed to eye injury while engaged in framing activities using a pneumatic nail gun, on or about 3/38/2019.   The Express Construction Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1079116, citation number 2, item number 1 and was affirmed as a final order on 09/16/2015, with respect to a workplace located at 1240 River Road Edgewater NJ 07020  The Express Construction Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1230898, citation number 2, item number 1 and was affirmed as a final order on 09/18/2017, with respect to a workplace located at Clifton Terrace Englewood Cliffs, NJ 07632  The Express Construction Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1200082, citation number 2, item number 1 and was affirmed as a final order on 04/03/2018, with respect to a workplace located at 590 Oak Ave Saddle Brook NJ 07663.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Building exterior East side: Employees were exposed to a fall hazard of 15 to 20 feet while installing plywood sheeting from the window seal on a new residential construction, without the use of personal fall arrest system or other means of fall protection in place, on or about 3/28/2019.  b) Building exterior North side: An employee was exposed to a fall hazard of 10 feet while installing plywood sheeting on a balcony of a new residential construction, without the use of personal fall arrest system or other means of fall protection in place, on or about 3/28/2019. 29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) Exterior of Building-Extension Ladder: An employee was exposed to a fall hazards of approximately 20 feet as the worker carried a full sheet of plywood up an aluminum extension ladder to the roof while engaged in sheathing activities, on or about 3/28/19.  b) Exterior of Building- Extension Ladder: An employee carrying a partial sheet of plywood on an extension ladder was exposed to fall hazards of approximately 15ft to the ground below while engaged in sheathing activities. Condition noted on or about 3/28/2019.
