DBA Fralo Industries, Batch Powder Coating Booth - On or about, January 4, 2017, and at times prior thereto: electrostatic hand-held spray guns were operable when the ventilation fans were not in operation. Employees conducted powder coating operations in the batch powder coating booth throughout each workshift.

DBA Fralo Industries, Supplied Air System, Batch Powder Coating Booth- On or about January 4, 2017 the Allegro Airline filter on an air compressor line, which was used to supply air to a supplied air respirator, was not tagged with the most recent filter cartridge change date. b.) The Victor Group, Inc. DBA Fralo Industries, Supplied Air System, Batch Powder Coating Booth- On or about January 4, 2017, the Allegro Airline filter on an air compressor line, which was used to supply air to a supplied air respirator, was not tagged with the signature of the person authorized by the employer to change sorbent beds and/or filters. The employer required employees to wear supplied air respirators during powder coating operations.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) The Victor Group, Inc. DBA Fralo Industries - On or about March 4, 2014, temporary employees worked with or around hazardous substances including, but not limited to, Surcoat 924, Fralo Red powder coating, Methyl Ethyl Ketone, and Fralo Smooth Red spray paint. Temporary employees were not provided effective information and training about hazardous chemicals in the workplace. THE VICTOR GROUP, INC. DBA FRALO INDUSTRIES WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, 29 CFR 1910.1200(H)(1) , WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 962429, CITATION NUMBER 1, ITEM NUMBER 6 AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 18, 2014, WITH RESPECT TO A WORKPLACE LOCATED AT 1651 E. 12TH ST. IN ERIE, PA.
