29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Fralo Industries, 1651 E. 12th Street, Erie, PA. On or about March 12, 2020, the back of the Wysong Shear was not guarded to prevent employees from being struck by the rear bar of the shear during operating. b) Fralo Industries, 1651 E. 12th Street, Erie, PA. On or about March 12, 2020, the hold-downs of the shear were not guarded to prevent the operator from having his fingers crushed by the hold-downs while operating the Wysong Shear. ABATEMENT CERTIFICATION REQUIRED:3 Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them: a) Murata Weidemann C2000 CNC Auto Index Turret, on or about March 12, 2020, the energy control procedures did not include specific methods to lockout the air hose. The procedure informed the authorized employee to disconnect the coupling and position the air hose so it cannot be easily reconnected. b) Murata Weidemann C2000 CNC Auto Index Turret, on or about March 12, 2020, the energy control procedures did not include specific methods to verify the isolation of the air/pneumatic energy source. c) Cincinnati Press Brake, on or about March 12, 2020, the energy control procedures did not include specific methods to deal with the potential/gravitational energy of the elevated ram by methods of blocking or positioning the ram to a Zero Energy State. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
