29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. On January 5, 2017, at the front of the project site, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter protection when employees used a 110 VAC Makita circular saw, model 5007NB, powered through an extension cord, to cut lumber, exposing employees to electrical hazards.
