29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: On or about January 4, 2017, the established Pasco Palletizer lock-out procedure (PASC000XLVW, revised 07/20/2016), was not used by employees to isolate the identified energy sources of electrical, pneumatic and mechanical. Employees were exposed to the hazards of unexpected energy release while performing routine maintenance on the palletizer."
