29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:       a. Thermoformer Series 2500 Stackers - On or about 11 January 2017, on each stacker of the thermoformer lines, the employees had were exposed to inadvertent contact with moving parts while required to remove product. This exposed employees to struck-by and caught-in hazards.    b. Wheeler Plastic Compactor waste dumpster lift - On or about 11 January 2017, the waste dumpster lift was not guarded to prevent employees from being caught-in and crushed by the dumpster bin when it is lowered. Employees walking and working the area were not guarded against accidental/incidental entry into such areas.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.332(b)(1): Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments:    On or about 11 January 2017, and on dates prior, employees were required to work on electrical equipment from 110-480 volts. Employees were  running 3 new electrical lines that were rated at 480volts. Employees were required to test the lines to ensure they were not live. Employees were not provided with the appropriate Personal Protective Equipment or tools to protect themselves.    The employees were not trained in and familiar with safety-related work practices, such as the use of special precautionary techniques, personal protective equipment (PPE), and insulating and shielding materials. The employees were exposed to shock and arc flash hazards.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees were running three new electrical lines that were rated at 480volts. Employees were required to test the lines to ensure they were not live. The employees were not capable of safely performing such work and were not trained in and were not familiar with safety-related work practices, including the use of special precautionary techniques, Personal Protective Equipment (PPE), and insulating and shielding materials. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards:     On or about 11 January 2017, and on dates prior, employees were required to work on electrical equipment from 110-480 volts. Employees were running three new electrical lines that were rated at 480volts. Employees were required to test the lines to ensure they were not live. Electrically insulated rubber gloves were not provided to prevent direct or indirect contract with live electrical parts. Employees did not employ other safety related work practices including adequate equipment and tools to protect themselves from electrocution and shock hazards. 29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed:    On or about 11 January 2017, and on dates prior, employees were required to work on electrical equipment from 110-480 volts. Employees were running three new electrical lines that were rated at 480volts each. Employees were required to perform voltage testing on the lines. The employer failed to provide electrical protective equipment  appropriate for the specific parts of the body to be protected when they were performed such tasks in areas where they had the potential to come into contact with energized equipment or encounter arc flash hazards.    Employees did not wear appropriate electrical protective equipment, including but not limited to the following, to protect against electrical shock injuries:      a) Rubber insulating gloves with leather protectors    b) Arc-Rated clothing such as arc-rated long-sleeve shirt and pants, or arc-rated coverall     c) Hearing protection (canal inserts)     d) Arc-Rated flash suit, hood or balaclava     Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).
