29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about 1/9/2017 at the facility, the employer failed to enforce the use of specific procedures by authorized employees expected to perform lockout on equipment including but not limited to the conveyor systems that are electrical and pneumatically energized. Employees performing lockout, did so by utilizing their own personal experience, were not aware of the location of, and did not reference the procedures that were created on 9/1/2015. b. On or about 1/9/2017 at the facility, the employer failed to create specific procedures for equipment including but not limited to the baler and Powered industrial trucks exposing employees to struck-by and caught-in hazards.

29 CFR 1910.178(l)(2)(i)(A): Trainees operated a powered industrial truck without direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence: On or about 1/5/2017 in Aisle #1 by the battery recharge area an untrained powered industrial truck trainee was left alone without supervision and suffered a fractured left leg when the lift struck another parked lift.
