On or about January 10, 2017, at De-Reeler #7 (Coil Handling System Serial # 04-5135 Model DR-120x36), the employer did not have specific procedures to lock-out and control hazardous energy when maintenance employees conduct services and maintenance. This condition exposed employees to struck-by hazards.
