Production Floor:  On or about February 06, 2024, employees were exposed to impact (eye) hazards in the following instances.  1) Press #3 Area, an employee was observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.  2) On press #5, an employee was observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.  3) Press #7 Area, an employee was observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.   4) Press #10 Area, an employee was observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.   5) Press #12 Area, an employee was observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.   6) Press #18 Area, an employee was observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.   7) Quality Control Room, an employee was observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.   8) Tool Room: Four employees were observed wearing prescription eyewear side shields, but the frames and lens were not American National Standards Institute (ANSI-Z87.1) rated safety eyewear.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Tool Room:  On or about February 06, 2024, the 8" Dayton Bench Grinder (Model 4Z909C, 3/4 HP-3450-RPMs), the distance between the adjustable guard exceeded one-fourth inch. The measured distance between the adjustable tongue guard and wheel was 3/4 of an inch exposing employees to struck-by hazards.
