29 CFR 1910.303(h)(2)(i): Electrical installations in a vault, room, or closet or in an area surrounded by a wall, screen, or fence, access to which is controlled by lock and key or other approved means, were accessible to unqualified persons. a) River Dock Area - On or about 1/10/17, employees were exposed to electrical shock hazards when the high voltage electrical room/vault was left opened/ajar to allow unqualified persons access to the room. The hazard was in plain view. With reasonable diligence, the hazard should have been recognized and abated.
