29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation.  a) On or about 1/11/2017, Nyrstar located at 1800 Zinc Plant Road in Clarksville, TN had not entered complete information on the 2016 OSHA 300 LOG, Column F:  1. OSHA 300 Log 2016 Column (f): cases 1; 2 and 3 did not include the object/substance that directly injured or made the person ill and/or the body part(s) affected.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  On or about 01/10/20117, Nyrstar employees working at 1800 Zinc Plant Road in Clarksville, TN were exposed to hazardous materials, diesel fuel and gasoline (eye, skin, and/or respiratory irritants) for which the employer did not compile a complete chemical list of all hazardous materials used in the work place.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
