It was reported by the employer that this event occurred on November 22, 2016.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: During the inspection on 01/17/2017, North Central Stamping, Inc., located at 920 Lonestar Drive, OFallon MO, it was confirmed that an employee was exposed to a horizontal hexagonal drive shaft on a Dorner parts conveyor which snagged his glove and caused an amputation injury. It was reported that this event occurred on November 22, 2016 but the employer only notified OSHA on January 10, 2017.
